Submit this form to the church office at least three (3) weeks prior to the requested date. All previously scheduled events take priority. The church office will respond via email or phone to confirm your request and explain any building use fees.

The applicant and person/s on whose behalf this application is made agree that they will jointly and severally protect, indemnify, and save harmless the First Church of the Nazarene, Indianapolis, Indiana, its members, officers, agents, and employees from every liability, claim, demand, right of action, loss, cost, damage, or expense (including attorney fees) on account of every injury to or death of a person/s or instance of damage to property which injury, death, or damage arises out of or is in any way connected with the use to which this application pertains. The use of tobacco and alcohol is not permitted in the facility or on the property. The applicant has examined the rules for use of the facilities and/or equipment requested. And agrees for him/her as well as for any other person/s on whose behalf this application is made, that the terms and conditions set out in this form shall apply to the use of which this application pertains.
